Impact of a Self‑Management Hypertension Program on Patient Health Outcomes in Rural Primary Care Clinics
The objective of my capstone project was to evaluate whether a structured, nurse‑led self‑management education program could improve blood pressure control and patient engagement among adults with hypertension in rural primary care settings. The project focused on empowering patients through education on medication adherence, lifestyle modification, blood pressure monitoring, and goal setting.
Key outcomes included improved patient understanding of hypertension management, increased adherence to self‑care behaviors, and enhanced nurse–patient communication related to chronic disease management. The project demonstrated the critical role nurses play in preventative care, patient education, and chronic disease management, particularly in rural and underserved populations.
Through this capstone experience, I strengthened my skills in evidence‑based practice, data interpretation, quality improvement, and interdisciplinary collaboration. I also gained a deeper appreciation for the importance of culturally competent education and sustainable interventions tailored to patient needs and community resources.

Feedback from clinical supervisors and peers has consistently highlighted my strengths in patient advocacy, professionalism, and commitment to safe, ethical nursing practice. Evaluations noted my strong assessment skills, attention to detail, and ability to communicate effectively with patients and healthcare team members. I was recognized for maintaining composure in high‑acuity situations and demonstrating accountability in clinical decision‑making.
Areas for continued growth identified in evaluations included developing confidence in leadership roles and expanding clinical efficiency as experience increases. I value constructive feedback and actively incorporate it into my practice to foster professional growth and improve patient outcomes.
